An application (free form) must be submitted, which includes:
1) a request regarding the change of the owner's data, the change of the representative or the change of the representative's data in the registration of the design, indicating the registration number, as well as the corresponding application number;
2) information which shall be amended in the Register of Designs, namely, the name and address of the registration owner or representative in the current Register entry;
3) the information to be included in the Register, namely the new name and / or address of the registration owner or representative;
4) One application is sufficient, even if the changes concern more than one registration, provided that the numbers of all relevant registrations are indicated in the application.
A power of attorney (original) shall be attached to the application if it is submitted by an authorized person. The power of attorney may be drawn up in a free form, but in essence it must comply with the general provisions of the Civil Law on the power of attorney agreement. If the applicant so wishes, he or she may use the power of attorney form drawn up by the Patent Office.
When submitting the application for a change of the data of the owner of the design, change of the representative or change of the data of the representative, the prescribed fee must be paid.(see Cabinet Regulation No.723, 15 December 2015, Price List of Paid Services of the Patent Office). -

If the application for change of the data of the owner of the design or change of the representative, or change of the data of the representative complies with the requirements of the Designs Law, the Patent Office shall make a decision regarding the inclusion of the relevant amendments in the Register.
The Patent Office shall make amendements in the information in the Register related to the change of the name and / or address of the registration owner, or the change of the representative, or the change of the data of the representative.
Information on the changes made shall be published in the Official Gazette.
An extract from the State Register of Designs regarding the change of data of the owner of the design shall be issued to the registration owner of the design; the extract shall be issued in paper or electronic format.
In case of doubt, the Patent Office may request evidence of the right to apply for a change of data.
